Format for Ban Request

When posting a ban request, please follow these guidelines:

1) Topic Title

Topic title should be in this format: Player Name - Player Steam ID

2) Post Body

Name: Player Name
Steam ID: Steam ID
Offense: Offense committed
Time: Approximate time you banned player, include your time zone.
Proof: Any proof or reasoning regarding why the player was banned.
